This departmental results report details the Office of the Correctional Investigator’s actual accomplishments against the plans, priorities and expected results outlined in its 2024–25 Departmental Plan.
Key priorities
The Office of the Correctional Investigator identified the following key priorities for 2024-25:
- Identifying and addressing disparate outcomes for Indigenous and Black incarcerated individuals with the goal of reducing their overall overrepresentation in Canadian prisons;
- Adjusting the organizational structure to improve effectiveness and resilience;
- Improving organizational capacity in planning and conducting systemic investigations and prison inspections;
- Optimizing information and data collection to better identify trends, the evolution of specific issues and to better report on success stories; and
- Strengthening the organization’s position as an employer of choice and thus ensuring “la relève” of the current and future key positions.

Summary of Results
The following provides a summary of the results the department achieved in 2024-25 under its main areas of activity, called “core responsibilities.”
Core responsibility 1: Independent Oversight of Federal Corrections

The Office of the Correctional Investigator (OCI) received and processed 4,352 inquiries about concerns pertaining to the Correctional Service of Canada during the reporting period. The OCI’s Investigations Team made recommendations for resolution of complaints and were successful in securing the Correctional Service of Canada’s commitment to address the issue at the source of the complaint 87.5% of the time.
The OCI conducted six national investigations, with a focus on access to and quality of mental health care for federally sentenced individuals. For these investigations, the OCI conducted a total of 425 interviews, both in custody and on community settings. The OCI conducted site visits, part of the 433 person days spent in institutions, and met with institutional and community staff, a variety of community-based stakeholders, Indigenous organizations, and provincial correctional authorities.
The OCI continued to monitor the response to recommendations resulting from its investigations. Although the recommendations to the Correctional Service of Canada are non-binding, it is important to note that 80% of the total number of recommendations were addressed in a manner beneficial to the targeted inmate population.
